A TARGETED, MINIMALLY INVASIVE TREATMENT FOR CELLULITES
Needle Subcision for Cellulite: Dubai Specialist
"Doc, what are the different treatments for cellulite? These dimples really bother me, and nothing has helped."
This is a conversation I have often in clinic.
The dimples may be on the buttocks or the back of the thighs. Sometimes there are only a few, but they are the ones you notice in fitted clothes, swimwear or photographs.
- "I have tried creams, massage and working out. These dents are still there."
- "It is not everywhere. It is just these particular dimples that I want to improve."
- Before I talk about treatment, I need to understand why those dimples are there.
Cellulite is not simply fat.
In many people, a small, firm band beneath the skin pulls the surface down. That is what creates the dimple.
That is why creams, massage, weight loss and liposuction do not reliably get rid of true cellulite dimples. They may change the fat or skin, but they do not release the band causing the pull.
For the right type of dimple, subcision can help.
What is needle subcision?
Needle subcision is a way of releasing the cellulite bands that are pulling a dimple down. I do this with a fine needle.
I usually do it in clinic under local anaesthetic. It normally takes 30 to 60 minutes, depending on how many dimples we are treating.
It sounds simple, but choosing the right dimples matters.
Once the area is numb, I slide the needle under the skin. I can use it close to the side of the cellulite band and release it. If needed, I gently move around the dimple to release the pull fully.
I am not removing fat.
I am releasing the pull that is holding the skin down.
After that, the surface can settle more smoothly. How much it improves depends on the dimple, your skin quality and whether there is also a hollow or volume loss.
Needle subcision or Avéli: how do I decide?
Both needle subcision and Avéli work in the same basic way: they release the bands underneath the skin that are pulling the dimples down.
The treatment we choose depends on the type of cellulite you have, the number of dimples, the area we are treating and what you are hoping to improve.
The practical difference is how we release the bands, where we do the treatment and what will be most comfortable for you.
The practical difference is how we release the bands, where we do the treatment and what will be most comfortable for you.
Avéli can be a good option when there are more dimples to treat. In my practice, I usually do it in hospital with local anaesthetic and sedation through a drip, because a larger area can be uncomfortable while you are awake.
Needle subcision can be a good option when we are treating a smaller area or just a few dimples.
I can numb the points I need to treat and do it in clinic. It saves you a hospital visit for a small treatment.
Why would I use a needle?
A needle lets me deal with one dimple at a time.
That is useful for a few separate dimples. I can place the needle beside the band and release the pull without treating more of the area than needed.
The entry point is like a pinprick, so it usually heals without a noticeable scar.
For the right person, it is a small treatment with a simple recovery.
Who tends to be a good candidate?
Needle subcision works best when a few dimples are caused by bands under reasonably firm skin. I commonly use it on the buttocks and back of the thighs.
When you come in, I look at the dimples while you are standing and moving. I need to work out whether I am seeing a band pulling the skin down, a hollow, loose skin, a general skin-texture issue, or a combination. That decides the plan.
When subcision alone is not enough
This is where I try to be very straightforward with you.
Releasing a band will not tighten loose or hanging skin. If your skin is thin, empty or sagging, the dimple may improve but the overall change may not be what you had in mind.
Then I may recommend something else: a buttock or thigh lift for significant loose skin, a collagen-stimulating treatment, or a staged plan. Sometimes I will tell you that subcision is not the right treatment.
Cellulite treatment works best when the skin has enough support after the band has been released.
How I plan cellulite treatment
Not everyone needs every step. When cellulite is more complicated, I usually think about treatment in this order.
First, release the bands. If a band is still pulling the skin down, adding volume or improving the skin first will not deal with the main problem.
Second, I check whether there is a real loss of volume.
Some areas still look slightly hollow once the band has been released. If that is the case, we can talk about temporary hyaluronic acid filler or staged volume correction with something such as Lenluma. Many dimples do not need this.
Third, improve skin quality if that is part of the problem.
If the skin is thin, crepey or lacks support, collagen-stimulating treatments such as hyperdiluted Radiesse, Sculptra or Lenluma may help over time. They cannot replace surgery for a lot of loose skin, but they can help in the right situation.
I put the plan together around your dimples, skin, any volume loss and the improvement I think is realistic.
Why I do not use permanent fillers for cellulite
Some places combine the release of cellulite bands with permanent filler.
I do not use permanent fillers such as PMMA, silicone or polyacrylamide gel for cellulite.
The reason is simple: I want you to have safe options in the future if something needs correcting.
Permanent fillers can be very hard to remove. They can move, become inflamed or form lumps, and fixing that can be much harder than treating the original cellulite.
My approach is to release the bands first. If there is still a hollow afterwards, I add temporary volume or a biostimulator only when it is likely to help.
Can subcision be done during body-contouring surgery?
If you are already having liposuction, thigh liposuction, a BBL or another body-contouring operation, I may be able to release selected cellulite bands at the same time.
During surgery, I may use a needle or a V-tip cannula - a small surgical tube - depending on the band and the area.
A V-tip cannula is stronger than a needle and can help when bands are firmer or the area is larger. I do not use it for every dimple.
This is different from needle subcision in clinic.
Cannula subcision can be uncomfortable when you are awake, so I use it only during surgery, with sedation or general anaesthetic.
Is needle subcision painful?
I do the treatment under local anaesthetic.
The numbing injections sting for a few seconds. Once you are numb, most people feel pressure, pulling or movement, rather than sharp pain. I work slowly and keep checking you are comfortable.
What is recovery like?
You can normally go home shortly afterwards and walk as usual.
Bruising, swelling, tenderness and a little firmness are expected. Bruising on the buttocks or thighs can look more dramatic than you expect, but usually settles over one to two weeks. Everyone heals differently.
I do not expect the treated dimples to look perfect straight away. The area needs time to settle.
If you have a lot of cellulite or several areas to treat, it can be better to do more than one session. I would rather treat you in stages than over-treat in one go.
Depending on the area and how much I have released, I may ask you to wear compression shorts or Spanx.

Why patients come to me for this treatment
My practice is dedicated to body contouring, so I do not look at a cellulite dimple on its own. I look at the skin, fat, volume, looseness and your proportions.
A dimple in firm skin is a different problem from a dimple in loose, deflated skin after weight loss.
Sometimes needle subcision is right. Sometimes Avéli is a better option. Sometimes a biostimulator makes more sense, and sometimes a lift is the more honest recommendation.
I see you myself and make the plan around your body: release the bands causing the dimples, avoid permanent fillers, add volume only when it is needed, and improve skin quality in stages when that will help.
I am not trying to make every patient fit one treatment.
I am trying to choose the treatment that makes sense for you.

Frequently Asked Questions
Is needle subcision the same as Avéli?
No. Both release the bands that pull the skin down, but they use different instruments. Needle subcision is manual and can be useful for a few dimples or a smaller area. Avéli can be better when there are more dimples to treat.
Can it be done in the clinic?
Yes. If the area is suitable, I can do needle subcision in the clinic under local anaesthetic.
Does it work for every type of cellulite?
Yes. Many men want less buttock projection and less hip roundness, especially when the lower body
looks too rounded for their frame.
Will I need more than one session?
No. It works best when a band is pulling the skin down. It will not tighten loose skin and may not be enough if the main issue is loose skin or a large hollow.
Can this be combined with surgery?
Possibly. A few clear dimples may improve in one session. More widespread cellulite, several areas, or a staged plan may need more than one visit.
Do you use permanent fillers for cellulite?
No. I do not use permanent fillers. If volume is genuinely needed after the bands have been released, I prefer a temporary filler or a biostimulator that we can use in stages.
Is the result permanent?
Once a band has been released, the improvement can last a long time. But skin quality, ageing, weight changes and untreated bands can all affect how the area looks over time.
Book a Consultation
If back rolls, bra rolls, upper back fat, lower back fullness, love handles or an undefined waistline are something you want to address, a consultation is the right first step. I'll assess the back alongside the waist, flanks, abdomen and skin quality, and recommend the right option — isolated back liposuction, VASER, J-Plasma, Lipo 360, revision, high-definition sculpting, or a back lift if loose skin is the main concern.
